Section 4. DESCRIPTION OF PLANNING, EVALUATION, REVISION AND APPROVAL PROCESS FOR ALL ELEMENTS OF THE PLAN OF SERVICE

A.  Plan Development

1)  Member Needs Assessment: What information was gathered and through what process (e.g. annual system evaluation, workshop evaluations, etc.)

The annual SLS evaluation form allows us to determine the viability of current System services on an ongoing basis. It also provides an opportunity for members to state annual goals for the upcoming year, and what services the SLS could provide to help meet those goals. Evaluation forms for each SLS workshops provide an additional opportunity for members to state their needs.

2)  Planning Process: Who was involved in the planning process? What was the role of member in developing the plan?

The new Plan of Service was discussed by membership at an SLS Staff Development day, and additions to service suggested. The SLS Council and SLS Liaisons further refined the draft plan at their meetings in the 2000-2001 year, with approval being given at the 3/7/2001 meeting of Councils. The finished Plan was posted on the web

for final comment and Council Approval in April 2001.

A.  Evaluation

1)  What information will be collected to evaluate whether or not the system has achieved the intended results of the plan?

The annual SLS evaluation mentioned above seeks feedback pro and con on all SLS services and goals.

2)  What methods will be used to determine whether the system's customers were satisfied with the system's services?

SLS members rate each SLS service on a five point scale on the annual evaluation. This information shall be used the Director and the SLS Council in evaluating the plan.

3)  How will the information on customer satisfaction be used to shape the system's plan in the next year or the following cycle?

Services will be added or deleted, and the Plan of Service modified appropriately, based on information gathered on the annual evaluation, surveys of membership, or other membership feedback tools.

A.  Amendment Process
